Health Insurance in the Czech Republic 2026: Public and Commercial Cover for Foreigners
Health insurance in the Czech Republic is not the same for every foreign resident. The correct type of coverage depends on residence status, employment, citizenship and in some cases EU or international rules.
The two main categories are public health insurance and commercial medical insurance. They have different legal bases, conditions and coverage arrangements.
Foreigners should confirm which system applies before relying on an old insurance card or a policy purchased for a previous residence status.
Who is covered by Czech public health insurance
According to VZP, public health insurance covers foreigners who meet statutory conditions, including many permanent residents and other groups covered by Czech, EU or international rules.
A third-country national without permanent residence may also enter the public system through qualifying employment with a Czech employer.
When commercial health insurance is required
Foreigners who do not qualify for public insurance generally need commercial health insurance. The policy must match the requirements of the person’s residence or visa procedure.
The Czech Ministry of the Interior explains that insurance-document requirements differ depending on the type of application and whether it is filed abroad or inside Czechia.
Health insurance and employment
If public insurance is linked to employment, starting or ending a job can affect insurance status. After changing employer or residence category, confirm the current situation directly with the insurer.
What changed in 2026
Separate rules changed from 12 June 2026 for some foreigners with international protection. VZP has published specific guidance for affected groups, including cases where commercial insurance may become necessary.
What foreigners should check
Check the type of insurance, validity dates, whether the employer is registered as payer where relevant, the scope of covered care and the document required when visiting a doctor. Residence changes should trigger a new insurance-status check.
